Historically, because of the similar dysphagia symptom, GCA has been categoried as esophageal cancer (EC), and in the past decades, GCA has not been well characterized in histopathological changes in these high risk areas. The present study was thus undertaken to analyse the histopathological changes in gross morphology, location, TNM staging and, especially the intestinal metaplasia (IM) in GCA from the patients in Linxian and nearby counties. 7301 cases with GCA were enrolled for this study, including 5621 males with a mean age of 59±9 (mean±SD) and 1680 females with a mean age of 58±9. Consecutive tissue embedding with paraffin was applied for the whole surgical specimen. Based on the gross changes, GCA was classified into three major types, i.e.: infiltrating, ulcer and fungating, and the percentage of the three gross types was 47%, 41%, 12%, respectively in male and 52%, 36%, 12%, respectively in female. There was no significant difference in gross type in gender. Histopathological examination showed that IM was identified in 26% of the GCA specimen examined. The predominant type of IM was type I (71%), followed by type II (23%), and type III (7%) (P 0.05). TNM staging showed that, of the 1485 GCA with clear TNM record after surgical resection, 42% in stage II, 55% in stage III-IV and less than 3% of GCA patients was diagnosed at early stage (TNM staging 0, I). Based on the chief location of GCA with relation to Z line, GCA was classified into different location types, i.e. type A (all the tumor was located under Z line without assaulting any esophageal squamous epithelium), type B (2/3 of the tumor under Z line), type C (half of the tumor under the Z line), type D (1/3 of the tumor under Z line), type E (all the tumor above Z line), and type F (indefinite, because of the Z line destroyed and replaced by cancer tissue). In the present study, the predominant location types (81%) for GCA was type A (33%) and B (48%), followed by type C (9%), D (6%), F (4%) and E (0.4%). The present studies demonstrate that the predominant anatomic gross type for GCA is infiltrating type, 26% of GCA occurs IM with a majority of type I, and 81% of GCA is located chiefly on the cardia side in Chinese patients, which indicates that more than 90% of GCA in Chinese may be #8220;true gastric cardia cancer#8221;, which is quite different with Western pattern in tissue origination and IM.Citation Information: In: Proc Am Assoc Cancer Res; 2009 Apr 18-22; Denver, CO.
